Please complete the application for the new service. Once you have submitted the application, a representative will process the application. If for any reason, we cannot fulfill your request promptly, a representative will contact you.

Applications are processed Monday through Thursday from 7 am to 5 pm, excluding holidays. An account set-up fee will be charged on the first bill.

The standard for starting service is the next business day or later. Same day service may be available for an additional charge to be billed on the first bill. Requests are processed between 7 am and 5 pm. Requests received outside these hours will be processed the following business day.
